If you are a contractor, casual employee, or some other party not directly employed by Paddle UK and need to report an accident as part of your work with us, please complete the Accident Book below.

You will be asked to complete details about the accident itself, and about the person who had the accident, if you were not that person.

Once completed and sent you should see a message that confirms your report has been logged. You should also receive a confirmation email with the details you uploaded sent to your email address. If you have any questions or issues please contact Samantha Ward here.
